New Patient Forms
Thank you for choosing TheraFUN! If you are interested in having your child seen for Occupational, Physical or Speech Therapy services, please contact your Primary Care Physician for a referral. All new clients will need to fill out the New Client Intake paperwork prior to being scheduled. We would love to help get you started in this process!

We are In-Network with the following insurances:

ZERO Health
For all other insurances not listed above, we can file for out-of-network benefits and provide an invoice/superbill for reimbursement. We also offer Private Pay rates for our therapy services. Please contact our office for more information on these options.
SOONERSELECT PATIENTS: Our clinic is currently registered with all SoonerSelect Health Plans. If your child is enrolled in a SoonerSelect plan and is a current patient or in the process of establishing services with our clinic, please text us a picture of the front and back of your new insurance card.
